Personal Care Assistant ll
Broad Statement of Responsibility
A person who provides supportive in-home services intended to maintain, restore and strengthen individuals in their own home, assisted living or nursing facility. Providing in-home care services in a client’s personal home may require you to work with clients who smoke, and/or have pets. You may have to drive/travel during high traffic times and in inclement weather. You must be able to follow a Service Plan and maintain a professional relationship at all times.
Qualifications:
1. Be at least 18 years of age.
2. Must be able to past an annual criminal background check
3. Must be able to pass a drug screening.
4. Follow the Agency’s dress code with regards to piercings, tattoos and hygiene.
5. Ability to lift 50 pounds.
6. Maintain a Current CPR card at all times.
7. Have at least one year of paid previous C.N.A. and/or personal care experience working under the direction of a registered nurse with at least one acceptable reference.
8. Must attend Medication Management Training provided by the Agency within 30 days of hire unless you have a current C.M.A. license and/or have attended an acceptable course in the last six months.
9. Ability to read and write the English language in order to follow the Service Plan and document accordingly.
10. Demonstrate ability to physically and emotionally respond to emergencies in a variety of client situations.
11. Ability to follow directions to carry out the individual Service Plans for assigned clients and follow the Agency’s Policy and Procedures.
12. Ability to work as a team member.
13. Proof of a valid driver’s license without restriction and auto insurance when the client requires transportation.
14. You must have a home phone, cell phone or personal pager at all times.
15. Willingness and ability to work during stated times of availability. This requires that you remain on call during the times you have listed, if not currently working.
Duties:
1. Follow the Service Plans developed by the Nurse Supervisor.
2. Provide personal care services, activities of daily living and all other services as directed in the Service Plan.
3. Assist with meal preparation, nutrition, light housekeeping, and shopping as directed in the Service Plan and perform other household services essential to health care at home as directed in the Service Plan.
4. Accompany client to medical or personal appointments as directed. May be required to run errands.
5. When instructed by the nursing supervisor, provide assistance with medications, and/or medication management services as directed in the Service Plan.
6. Utilize appropriate durable medical equipment as instructed by the nursing supervisor.
7. Ability to work professionally with both Home Health Care and Hospice staff.
8. Assist with all physical therapies as directed by health care professionals.
9. Follow all delegation procedures as directed by the nursing supervisor.
10. Communicate immediately with supervisory staff on changes in client’s medical condition, all medical emergency situations, and any other incidents or changes necessary to the Service Plan.
11. Report all incidents immediately to the supervisory staff and complete proper documentation.
12. Communicate immediately with supervisory staff on ALL scheduling changes.
13. Complete appropriate records to include Documentation of Client Services Form, Time Cared and have them in the office
NO LATER THAN 5 PM EVERY MONDAY.
14. Complete Availability Sheet every week if your availability changes. You must be available to work during the times you have listed and therefore, must agree to remain on-call.
15. Comply with ORS 410.610 in reporting elder abuse and neglect to your supervisor.
16. Work with family, client and fellow employees in an understanding and professional manner.
17. All other duties as assigned by the nursing supervisor.
18. Complete 6 hours of continuing education per year.
