Certified Nursing Assistant                                                                                      

Broad Statement of Responsibility
Certified Nursing Assistants provide supportive services intended to maintain, restore and strengthen individuals in their own homes. These services may include but are not limited to personal care and grooming, performing simple procedures as assigned by the RN, assisting with medications that are ordinarily self-administered, reporting changes in the client’s condition and needs and completing all appropriate records and reporting regarding nursing care to the supervisor in compliance with the Service Plan and State Regulations.

Qualifications:
1. Be at least 18 years of age.
2. Completion of a recognized certified aide program and proof of registration and be in good standing with the
    Oregon State Board of Nursing.
3. A minimum of two acceptable references.
4. Work experience as a CNA with at least one year verifiable experience.
5. Ability to read and write the English language in order to follow the Service Plan and document accordingly.
6. Demonstrate ability to physically and emotionally respond to emergencies in a variety of client situations.
7. Ability to carry out the individual Service Plan for assigned clients.
8. Ability to work as a team member.
9. Proof of a valid driver’s license without restriction and auto insurance when client’s auto is used as transportation.
10. Ability to lift 50 pounds.
11. Current CPR card.

Duties:
1. Communicate on a regular basis with supervisory staff.
2. Comply with State Regulations.
3. Comply with agency policies regarding timely submission of personnel documents.
4. Comply with ORS 410.610 in reporting elder abuse and neglect to your supervisor.
5. Follow Service Plan for individual client and report any discrepancies to supervisor.
6. Complete appropriate documentation of client’s status and condition as directed by the Nurse Supervisor.
7. Work with family, client and fellow employees in a compassionate and understanding manner.

Medical Record and Documentation Guidelines:
1. Medical records are legal documents. Record everything in black or blue ink.
2. All documentation is to be recorded on a “Documentation of Client Services “ form.
3. “Documentation of Client Services” forms are to be turned into the office weekly (due by 5pm Monday), along with time cards.
4. Each CNA is responsible to keep a supply of “Documentation of Client Services” forms. You may stop by the office to pick them up,
     copy them yourself, or request that some be mailed to you.
5. Each CNA is responsible to the Nursing Supervisor for nursing problems. Examples of nursing problems are: care of client, care of
    equipment, and reporting unusual observations such as an increase in temperature or loss of weight.
6. STAFFING NEEDS ARE TO BE HANDLED THROUGH THE OFFICE. If you are unable to report for work call the office 4-6 hours before
     your shift begins. It is not appropriate to call another caregiver to cover your shift. You must contact scheduling coordinators.
7. Service Plans are to be used by all staff. If the Service Plan becomes outdated due to a change in the client’s condition, the Nursing
    Supervisor must be notified so that it can be revised to accurately reflect the client’s needs.
8. It is important to report to work early enough to get report if you are relieving another caregiver.

Work Environment:
You may have to work with clients who have infectious diseases, smoke, and/or reside in an unclean environment. You will be expected to use appropriate precautions at all times. You may have to drive/travel in inclement weather. You may have to climb stairs and perform transfers within reason.

Dress Code
Neat, clean, well maintained clothes are appropriate. The client may request specific attire. Closed toed shoes are required for safety.
